Driver’s License/Certificate of Attendance Application

(Must allow five days to process each request) Tennessee law requires every school or school district to verify that a 15-, 16-, or 17-year-old applicant or holder of a Tennessee driver’s license has met the compulsory attendance and academic requirements. Students must request a certificate of attendance from the main office to apply for their permit at the DMV. In order to receive this form, students must meet both of the following requirements:

  • A student must not have 15 or more unexcused absences in a term or 10 or more consecutive unexcused absences.
  • A student must have a passing grade in at least one-half of the full unit subjects for which he or she is taking (3 credits).

Revocation Any student fifteen (15) years of age or older who becomes academically deficient or deficient in attendance shall be reported to the Department of Safety for driver’s license revocation. A student shall be deemed academically deficient if she/he has not received passing grades in at least two (2) full unit subjects or their equivalency at the end of term grading.
